Editing a risk assessment survey

The following roles can edit a survey:

  • Anyone who is a member of the survey.


Surveys can be edited in Hyperproof or outside of Hyperproof in CSV format. The steps below explain how to edit a survey directly in Hyperproof.

  1. From the left menu, select Assessments.

  2. Select the Risk Surveys tab.

  3. Select the survey you want to edit.

  4. Optionally, to change the name of the survey, hover over the current name, then click the Edit icon.

  5. In the right pane, click View survey.

  6. Click Edit.

  7. Do any or all of the following:

    1. Click New to add a new section, question, or text box.

      Question types:

      • Single-select - A drop-down list of values where only one value can be selected.

      • Multi-select - A drop-down list of values where multiple values can be selected.

      • Single-select with explanation - Same as single-select, but with space for optional free text.

      • Multi-select with explanation - Same as multi-select, but with space for optional free text.

      • Open text - Free-form questions that allow the respondent to answer in an open-text format.

      • Proof upload - The only required answer is an attachment (of proof).

    2. Rearrange questions by dragging and dropping them.

    3. Rearrange sections by dragging and dropping them.

    4. Expand a question box to:

      1. Edit the question and/or answer options

      2. Change the question type

      3. Make the question optional or required

      4. Make the question a gating question

      5. Duplicate the question

      6. Delete the question

  8. Click Save.
